SETMA has been committed to excellence in long-term residential care for Southeast Texas residents since its founding in 1995.. Now, a new generation of SETMA providers takes up the mantle and renews SETMA's commitment. The I-CARE team has been organized to give more structure, substance and support to the care of our patients in long term residential centers and to the professional and administrative staffs of all of the facilities in which SETMA's team works.
Currently, SETMA's I-CARE team includes nurses, technical staff, nurse practitioners and physicians, all of whom have a special interest in the care of geriatric and disabled patients who require residential and ambulatory care.
Over the past fifteen years, SETMA has designed and maintained tools for the support of this interest. These tools include:

- A specially design suite of Nursing Home templates addressing the unique needs of long-term, residential-care patients, including assessment tools for:
- A level-two reference laboratory
- Access to laboratory results for nursing home patients
- Mobile X-ray and EKG services
- 24 Hour a Day, Seven days a week response to emergent needs
- A newly deployed I-CARE.com e-mail access for communication with the I-CARE Team.
- A community-wide Health Information Exchange, which is presently connected with one hospital but within the year will be connected with all nursing homes, hospitals, medical practices, Altus Hospice and other healthcare agencies.
- NextMD which is a secure web portal which will supplement the capacities of SETMA's website
- A hospital, inpatient support team which provides "transitions of care" documents to nursing homes when their residents return from the hospital with
- Complete orders for nursing home care
- Reconciled Medications
- Detailed summary of hospital course and testing performed
