What is HIPAA?

HIPAA (Health Insurance Portability and Accountability Act) was enacted as a broad Congressional attempt at incremental healthcare reform. The area of HIPAA that most patients are interested in is that of privacy. The privacy portions of HIPAA require that we only disclose PHI (Protected Health Information) to a pre-defined list of people and organizations. These organizations include the patient themselves, payers, other providers involved in treatment, public health organizations, state and federal agencies, etc.

SETMA has spent many hours training it’s employees on how to protect your information and why it is so important. If at any time you feel your privacy has been violated you have the right to report it to the proper authorities. However, we certainly hope that you will give us the opportunity to respond to your concerns and to take whatever steps necessary to resolve the situation.

What is Patient Consent?

The Patient Consent and Acknowledgement form allows SETMA to use your protected health information by your physician, our office staff and others outside of our office that are involved in your care and treatment for the purpose of providing health care services to you. Your protected health information may also be used and disclosed to pay your health care bills and to support the operation of the physician’s practice.
