As a part of our medical home initiative, SETMA assists patients in the effort to find resources to help pay for healthcare. Included in this are:

  • SETMA Foundation - SETMA has a 501-C3 Foundation which helps pay for healthcare costs which the patient cannot bear. In 2009 alone, the partners of SETMA contributed $500,000 to the foundation. The Partners of SETMA, SETMA's CEO, COO and a community representative form the Foundation's Board. A committee made up of SETMA's CEO, COO and a community representative review significant requests for support. Support for $300 can be made by either the CEO or COO.
  • Assist patients in applying for Medicaid.
  • Assist patients in applying for Social Security Disability.
  • Refers patients to public health clinics for preventive health services to minimize their cost of care

Freestanding nurse-managed clinic that offers comprehensive care to underserved, especially those residing in the south end of Beaumont. The clinic offers acute, chronic, and preventative health care using a nurse practitioner model.

They provide the following services:
Physician services, Inpatient/outpatient hospital care, 3 Prescriptions per month , Skilled nursing facilities, Family planning services , Lab & x-ray services

Information needed to apply:
Social security card for all household members, Proof of identification, Proof of all household income, Proof of residency , Proof of resources

Eligibility is determined within 14 days of application and there is assistance available in preparing the application form.
