The Notice of Privacy Practices form is SETMA's explanation of our commitment to you, our patient, to protect your information from improper use.
It states that we keep your information secure and that we will only use it for one of three purposes.
- We will use the information within SETMA to deliver the best possible healthcare to you.
- We may share it with a company that works for us. Examples of this would be a transcription service, claims processing service or a radiologist to examine an X-ray.
- We may share it with others only when necessary to perform a service for you or if SETMA is legally required to do so.
SETMA does not share any customer information with third-party marketers.
